Vital Records is a core public health function that collects and maintains data on births and deaths. This information helps track health trends, guide programs, and support planning and evaluation. The Person County Vital Statistics Program ensures accurate and complete records are available for future use.

The Health Director serves as the Local Registrar, supported by a Deputy and Assistant Deputy Registrars.

Registering Births & Deaths

Hospital Births: The hospital or facility must file a Certificate of Live Birth within 10 days in the county where the birth occurred.

Deaths: A funeral director (or responsible person) must file a Notification of Death within 24 hours and a Certificate of Death within 5 days.

The Vital Records Section registers all births and deaths in Person County and issues:

  • Burial Transit Permits
  • Disinterment/Re-interment Permits

Local registrars ensure all records are properly filed with both the Person County Register of Deeds and the NC Vital Records office.

Home Births

A Certificate of Live Birth for a baby born at home (home birth) must be filed through the Vital Records Program within one year of the child’s birth.

The following documents are required for appointment:

1. Fill out the Home Birth Registration Packet completely (no blanks) and return by mail, fax, or email to:

Person County Vital Records
355 A South Madison Blvd.
Roxboro, NC 27573
Fax:  336-597-4804
Email: bpettiford@personcountync.gov

2. Obtain a statement from someone who can verify the mother was pregnant. This can be a coworker, pastor, neighbor, friend, or anyone who is not a relative. This document must be notarized (you may use the Home Birth Pregnancy Affidavit). You can also use a copy of an ultrasound if it has a date and the mother’s name printed on it.

3. Obtain a recent document that verifies your residence (utility bill, tax record, etc.).

4. Valid picture ID for both the mother and father (if the father’s name will be added to the certificate)

You will receive a call from our office to make an appointment to come in and complete the registration of your home birth. The mother may not be the only person required to be present. The father may also be required if his name is to be added to the certificate.

If you have questions, please call 336-597-2204.
